9b75d09a7a Core/Spells: reworked and fixed Combat Resurrection system 
* use SPELL_FAILED_IN_COMBAT_RES_LIMIT_REACHED to notify resurrecting player if no resurrection charges are left
* removed mechanics that are not used for Cataclysm
