Tools/MeshExtractor: Implemented mmap mesh generation for instances (They do not have ADT files, they're just a big WMO).
Cleaned up some things and fixed some errors.
ToDo:
* Test this a bit more.
* Cleanup the codebase for this, remove duplicated code.
